Referral Process

Pet Owners


London Regional Veterinary Emergency and Referral Hospital operates on a referral and emergency basis, and does not provide primary veterinary care such as vaccinations, spays and neuters, or preventive veterinary healthcare. Access to our specialty services is through a referral from your primary care/family veterinarian. The Emergency Department welcomes patients 24 hours a day, 7 days a week, 365 days a year. Emergency or overnight care being transferred from your family veterinarian requires a referral from your primary care veterinarian.  If you have questions regarding the referral process, please contact the office directly at (519) 432-3300 or by e-mail at

Referring Veterinarians


Referral Forms may be downloaded here or faxed on request. We request that the following documents are provided a minimum of 48 hours prior to your client’s appointment, where possible:

  • Referral form with relevant history (full medical history generally not required)
  • Laboratory results including cytology/histology
  • Pertinent radiographs

Digital radiographic images can be sent by e-mail in advance, or saved on a disk and sent with clients. Photographic images from film radiographs can also be sent by e-mail
